Shift: Monday - Friday 8am to 5pm
Location: Daytona Beach, FL
Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.
Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.
Communicates through various digital channels to support patient care. Performs initial health screens and intake for new members.
Obtains and reviews old medical records to ensure comprehensive patient history. Identifies care gaps and orders necessary screening tests.
Confirms appointments and provides instructions to patients.
