Job Summary: This position helps to navigate patients through the transplant process, including providers' offices, hospitals, patient-support organizations and other components of the health care system. In addition, the Patient Navigator helps to identify barriers that may prevent patients from receiving timely and high-quality healthcare treatment and working to find solutions to those barriers.
Responsibilities:
- Work with Transplant Center Staff to prioritize and coordinate services needed for this patient population. Become knowledgeable about community and medical resources that may be helpful to patients
- Assist in overcoming logistical barriers to scheduled diagnosis and treatment appointments.
- Contact patients who are "at risk" for missing appointments; coach them on the importance of obtaining care
- Provides educational materials as indicated.
- Ensure that appropriate medical and other collateral information is available in the patient's medical records during scheduled appointments.
- Facilitate linkages to supportive services such as transportation, lodging and other services as needed
- Establish a strong rapport with patients; coach them to become advocates for their own care . Build awareness of patient navigator services among the health care team by building professional relationships.
- Provide information about patient navigator service and maintaining communication to identify patients who are "at risk" barriers to care. Escalate barriers to appropriate team members for resolution.
- Communicate with providers about unique patient needs, (i.e., cultural).

Qualifications:
Experience: Two years of experience in healthcare, required. Knowledge about health care systems, the breadth and accessibility of community resources and working knowledge of HIPAA regulations, preferred.
Education: High school diploma or equivalent, required. Associate’s Degree, preferred.
