UNC Health Medical Group is looking for a Per Diem Advanced Practice Provider (NP/PA) to join the growing UNC Waynes?s Post Discharge Transitions of Care Clinic as a virtual APP. If this is the position for you, please apply!

Responsibilities:
Provide remote patient management and care, collaborating with healthcare providers.
Utilize electronic medical records (EMR) systems to facilitate communication and care delivery.
Manage patient communication, including scheduling appointments and sending reminders.
Facilitate video consultations and ensure timely responses to patient inquiries.
Schedule:
(Two or more days a month)
Hours: 8:00 a.m. - 4:00 p.m.

Qualifications:
Graduation from an accredited NP or PA school
One or more years of full panel primary care experienced required
North Carolina licensed
BLS & ACLS certified
