40 hrs/wk; 5 days/wk. 8a-4:30p, every third weekend and rotating holidays. Coordinates the Senior Behavioral Health intake process and function as direct liaison with the Healthcare team through patient referral, inquiry, admission, treatment and discharge process. Coordinates treatment with providers and other health and social agencies. Facilitates supports, provides group, family and individual sessions, and completes documentation. Provides appropriate support to patient and families. Utilizes appropriate assessment tools such as agitation and Mental Health Exams. Participates in development of treatment plan. Cross train and provides coverage for Discharge Coordinator and Activity Program Coordinator.
MSW,LMSW, PMSW, LMPH or LIMPH, with supervision. Experience in Behavioral health background preferred.
