Incumbent serves as the Mental Health Residential Rehabilitation Treatment Program (MHRRTP) Senior Social Worker, a member of a multi-disciplinary treatment team within the MH Service of the WTVAHCS. Incumbent reports directly to the Chief, MHRRTP, primary assignment is the MHRRTP which provides residential treatment with a primary purpose to provide clinical social work services in the MHRRTP. A Recruitment/Relocation Incentive may be authorized for a Highly Qualified Individual. Duties include, but are not limited to: Patient Care Independently planning effective treatment through the provision of a biopsychosocial assessment and developing a treatment plan Assessing and completing an advance directive for assigned patients Using known available resources and initial assessment of the client's readiness for change; making initial and continuing decisions regarding the agency's services and referrals Independently concluding the appropriate action, even in instances where agency actions can have serious impact on the life of the person served Providing full range of clinical social work services: case management, individual therapy, group work, coordination of agency and community support services, and other major treatment modalities to address client problems Facilitating optimal social functioning of assigned clients Authorizing benefits Giving advice, guidance, emotional support and other assistance Developing social work discharge plans for assigned patients Establishing a continuing relationship with the client, evaluating progress towards goals, performing follow-up after referral or if the client misses a session, and adjust treatment plan if results are inadequate Monitoring client's progress Maintaining comprehensive case documentation Work Schedule: Monday thru Friday 8:00 a.m. to 4:30 p.m. Financial Disclosure Report: Not required This is a Bargaining Unit position
