The Senior Social Worker is assigned to the Emergency Department to provide clinical psychological assessment and treatment at an advanced practice level to eligible Veterans and their family members. The incumbent provides a variety of services for Veterans referred from the Emergency Department including psychosocial assessments, treatment planning, short-term, solution focused counseling with individuals and families in crisis who may need various resources and post-intervention follow-up. Major duties for the Emergency Room Senior Social Worker include (but are not limited to): Providing case management, networking with community resources and coordination of care with the mental health team Making rapid assessments and develops crisis management plans for admission to mental health and/or medical inpatient programs Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills Educating patients on wellness/prevention education, assisting patients and their significant others with coping/dealing with the loss and grief experiences in disability, terminal illness and death Modelling effective Social Work practice skills, teach or providing orientation or internship supervision to less experienced Social Workers Addressing Advanced Directives and Organ Donation in accordance with the standards of the medical facility Placing, receiving and completing consults and orders to/from interdisciplinary teams and other Social Workers on complex difficult cases using advance practice and expertise Collaborating in the performance improvement processes and comply with performance measures as required by the VA for their specific population and/or program Communicating effectively orally and in writing with people from varied backgrounds.
