| The Cincinnati, OH VA Medical Center is recruiting for a Nurse Practitioner (NP) to work in the Women's Health section of Primary Care. This NP is accountable for the management and evaluation of a panel of patients under their care. The NP is a member of the Teamlet providing diagnosis and treatment Work Schedule: primarily M-F 8:00 - 4:30 pm with 2 Saturdays per month and may include some evening hours This is a temporary position, not to exceed one year. Condition of Appointment: This position is funded for one year. This appointment does not confer status as a VA employee for purposes of continued employment. At the end of the not-to-exceed appointment, your employment with the Cincinnati, OH VA Medical Center will end unless additional funding is granted to extend the appointment. If additional funds are approved, this appointment may be extended or possibly converted to permanent without further competition The NP is prepared in advanced practice across the health continuum, and the role includes participating in and use of research, development and implementation of health policy, leadership, education, case management, and consultation. Duties and responsibilities include but are not limited to the following: Provides comprehensive preventative and wellness care, as well as, the spectrum of acute, episodic, chronic, and end-of-life care to Veterans assigned to their Patient Aligned Care Team (PACT) Teamlet. Uses the nursing process and evidence-based practice to work collaboratively with the Teamlet and the family/caregiver, internal and community-based services and disciplines involved in providing care to the patient, and in developing the patient-driven holistic care plan for life. Interviews, evaluates, assesses, diagnoses, and treats ambulatory patients presenting to the Primary Care Clinics. Utilizes preventative clinical guidelines, appropriate nursing and medicine standards of care, and collaborates with team physicians in providing quality care to patients presenting with acute and chronic conditions. Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. Utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacological and non-pharmacological treatments in the direct management of acute and chronic illness and disease. Promotes wellness and prevents illness and injury. Serves as a resource to other members of the health care team, providing consultation and education as needed. Demonstrates leadership through development and coordination of programs to improve quality of care and organizational processes. Demonstrates and maintains full competency in the use, pre-cleaning and/or reprocessing of reusable medical equipment. The NP conducts a comprehensive assessment of patients' medical, physical, and bio-psychosocial status including: Obtains a relevant health and medical history Performs a physical examination based on age and history Conducts preventative screening procedures based on age and history Updates evaluation of health status to determine need for further health care Formulates appropriate medical diagnosis based on data obtained Orders appropriate diagnostic procedures and laboratory tests Identifies and prescribes appropriate pharmacological and non-pharmacological agents Identifies needs of the individual and family as a result of the evaluation of collected data Provides relevant patient/family education Consults with collaborating health care professionals when doubt exists as to the diagnosis, when expected improvement is not soon apparent, and when specialized therapeutic or diagnostic techniques are indicated Determines the effectiveness of the plan of care through documentation of client care outcomes Reassesses and modifies the plan as necessary to achieve medical and health goals Utilizes Telehealth programs in providing care coordination throughout the continuum of care Performing any procedures for which she/he has been credentialed The Nurse Practitioner, Primary Care, is accountable to the ACOS/Primary Care Service. They work in collaboration with the attending staff, Primary Care Nurse Practitioners, and physicians. The Nurse Practitioner is directly supervised by the Primary Care Service Nurse Manager or Chief Nurse assigned to their assigned location.
