This Hospital in Home Registered Nurse position is located in the Medical Service at the West Roxbury campus location. This position is a full-time position. Hospital in Home is a program that provides acute level clinical care to veterans in the comfort of their home 7-days a week, with the goal of avoiding admission to the hospital or reducing length of stay once admitted. This program is supported by a multidisciplinary team that provides visits, as often as daily, with both in person and virtual visits. The direct care RN must be a clinical expert who is competent in triage and assessments, EKG and telemetry, IV and phlebotomy skills, IV medication administration with familiarity of diuresis among other medications, wound care, and technical skills to utilize virtual capabilities. The direct care RN must be autonomous and able to work independently in the filed with minimal resources and direct oversight, with excellent time management skills and the ability to multitask. Duties include, but are not limited to: Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling. Provision of care supportive to or restorative of life and well-being. Executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. Work Schedule: Day Shift with Weekend and Holiday Rotations, Hours to be set between 8:00 am and 6:30 pm Financial Disclosure Report: Not required Compressed/Flexible Tour: Authorized Telework: May be Authorized Virtual: This is not a virtual position Relocation & Travel Incentives: Not Authorized Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. Notifications: This position is a NAGE Bargaining Unit position. This position is covered by a special rate. Selectee may be required to work at any VA Boston Healthcare System campus, as needed. The incumbent may be required to travel to other VA campuses and CBOC's. Current and former Federal employees must submit copies of their most recent SF-50 (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. This position is in the Excepted Service. The salary is based on the grade and step approved for the selected candidate. The Professional Standards Board (a peer-review group) will recommend the grade and salary for new appointments based on the candidate's education and experience. For internal selectees being promoted, the board will determine grade and HR will determine step in accordance with policy.
