Our busy Intake Department is seeking an Assessment and Referral Counselor, for our overnight shift, (11:00 PM to 7:00 AMwho will be responsible for conducting assessments of incoming patients and responding to requests for services and information. Making, documenting and processing inquiry calls in accordance with Intake policies,
Conducting assessments on individuals experiencing crises,
Participating in screening patients for admission into both inpatient and outpatient hospital programs, utilizing pre-approved admissions criteria set forth by the Medical Staff,
Providing assessments of incoming patient's mental health/chemical dependency; including precipitating events, presenting problems, treatment/substance use history, mental status and risk assessment,.
Making a determination of level of care by an evaluation of the patient's needs, advocating the lowest level of care consistent with those needs.,
